Self-Care Pamphlet in Illness:
Angiography
Confirmed by: Dr. Afshani
The general goal of educating the patients is that, while familiarizing themselves with their disease, they are able to make the right decision regarding daily activities, taking medications, and self-care after discharge.
Definition of coronary angiography (blood vessels of the heart):
Coronary angiography means imaging the blood vessels of the heart by injecting contrast medium. With this procedure, the doctor knows whether the heart vessels are blocked or not.
Angiography is carried out through the groin or the wrist, which does not require anesthesia to perform this diagnostic procedure and is performed with local anesthesia. This usually takes between 15 and 45 minutes.
Angiography is one of the most accurate diagnostic procedures in cardiovascular diseases.
Angiography is performed to:
* Confirm or assess the presence of heart disease (such as coronary artery disease, heart valve disease or aortic disease).
* Evaluate the function of the heart muscle.
* Specify the need for further treatment (such as angioplasty or bypass surgery).
Measures before angiography:
· Take a bath the night before going to bed and shave your extra hair from the navel to the knees, chest (for men), armpits and both hands.
· Be sure to inform your doctor about the history of any disease, such as blood clotting diseases and medications.
· On the morning of the operation, take blood pressure and heart control medicines with a little water.
· Inform your doctor and nurse if you have any drug or food allergies.
· Avoid eating and drinking 6 to 8 hours before angiography.
· Stop taking blood thinners such as Warfarin, Oxaban, Rivaroxaban and Apixabant hree or five days before the operation.
· Do not take blood sugar control drugs, Metformin, Ziptin, Sitagliptin, Zipmet, and Glucophage from the day before, and do not take all diabetic drugs on the day of operation.
· Avoid wearing jewelry.
· It is mandatory to have someone with you as a companion on the day of surgery.
· If you have already had open heart surgery, bring the heart surgery description with you.
· Bring all medical documents such as echocardiography, electrocardiogram, exercise test, nuclear scan and tests.
Training the patient about post-angiography care:
1. You should not get out of bed 6 to 8 hours after angiography.
2. If you have an angiography through the groin, a bag containing sand is placed on the groin to prevent bleeding in this area. Due to placing a sandbag and lack of movement, you may experience mild backache. You can move the other leg a little to reduce the pain.
3. If you have an angiography through the wrist, avoid bending, strong movement, pressure or heavy activity with the wrist.
4. Don’t raise your head and neck and don’t put your hands under your head.
5. Don’t cross your legs.
6. If you sneeze, cough or laugh, press the bandage to prevent bleeding.
7. After the surgery, you cannot eat anything by mouth until you are allowed. It is recommended to use enough fluids after starting the diet to eliminate the contrast medium faster.
8. In case of symptoms such as chest pain, asthma, sweating, nausea and vomiting, inform the nurse immediately.
Post-angiography care at home:
1. Take the CD and angiography result from the ward and be diligent in keeping it.
2. You are allowed to take a shower after discharge, but it is recommended.
3. Take your medicines on time and in the amount prescribed.
4. Follow a low-salt and low-fat diet.
5. If you have a history of blood sugar or blood lipids, you should control yourself with diet and medication according to your doctor’s opinion.
6. Avoid extreme emotions, stressful and crowded environments.
7. Avoid smoking.
8. Before discharge from the hospital, the dressing of the angiography site is removed and there is no need to re-dress it. But it is necessary to keep it clean and dry (avoid applying creams, herbal substances, etc. on it).
9. It is recommended to avoid driving for a long time, and if you have to drive a long distance, clean the car every 2 hours, take a walk, stretch your legs and rest them.
10. Consult your doctor if you are traveling by a plane.
11. Be sure to return to the doctor’s office at the specified time after discharge.
12. Avoid doing heavy exercises and pulling and pushing objects (especially if you have had an angiography through hand vein) for a week.
13. Avoid running, climbing or walking uphill or downhill for a week.
Note: If the angiography site becomes inflamed, bleeding, or swollen, while staying calm, lie down and apply firm pressure on the surgery area for ten minutes, and see a doctor or go to a medical center as soon as possible.
* If any of the following symptoms appear, see your doctor as soon as possible:
v Moderate to severe pain at the angiography site or pain in the limb that angiography was carried out on it.
v A feeling of warmth at the angiography site, which can be the cause of bleeding.
v Feeling of weakness, cramping or numbness in the limb that angiography was carried out on it (angiography through hand or foot)
v Redness and discharge at the angiography site
v Fever over 38 degrees
v General feeling of coldness and lethargy or feeling of coldness and tingling in the affected leg
v Skin lesions
